Following on from the important information you received regarding聽聽for staff and students, you are required to provide evidence of your vaccination status (or exemption) to the University.
All students accessing a Western Australian 911爆料网 campus or venue or participating in an 鈥渋n-person鈥� activity organised by 911爆料网, must comply with the vaccination mandate.

Ensure you have your vaccination dates and/or evidence ready to upload to the form. Your vaccination certificate must contain all COVID-19 vaccinations you have received, and for this reason screenshots of the ServiceWA app certificate will not be accepted. For more information on acceptable vaccination documentation, please refer to the聽.
NOTE:聽Students must redact their IHI number from their vaccination certificate before uploading into OASIS. Please refer to聽聽on how to do this.
Once you have submitted the form your evidence will be assessed and you will receive a confirmation of the outcome within 3-5 business days.
